GKNMH Home care services – A Community based Program
The population of patients suffering from chronic diseases is on the rise in the developing countries as are their medical expenses. Chronic diseases are progressive long-term medical conditions like heart diseases, chronic respiratory diseases, stroke, chronic renal failure, cancer etc. These diseases limit the capacity of the patient to perform everyday tasks like personal grooming, mobility and household chores; and they increasingly need assistance. They are major drivers for seeking repeated consultations and consequent hospitalization.
Studies have shown that an in-home care delivered by trained nurses guided by physicians has a beneficial effect on health outcomes such as reduced morbidity, unplanned hospitalizations and emergency department visits. In-home visits, in addition, improved activities of daily living when guided by physiotherapists and occupational therapists.
In Coimbatore, GKNMH has pioneered home care service as a continuum of care for its patients since 2002. Well appreciated within the community, this service - provided by the departments of Geriatrics and Palliative Medicine - was available on a need-only-basis. Our recent expansion of home health care services has made it accessible for most patients suffering from chronic debilitating illnesses rather than just a few.
The services provided under home care program will include, but not limited to, discharge planning, in-home care guidance, continuity of care, priority scheduling, screening for depression/ anxiety, self-management support.
