The Receptionist will hand over the “Outpatient Registration Information Form to the patient/attender to fill in the patient’s demographic details which is digitalized. The system automatically generates a unique identification number and a bar-coded Outpatient Card (i.e. smartcard) is issued which is used throughout his/her association with G.
