  • The receptionist will appraise the patient on the doctor’s availability and their timings. The patient will be registered to under doctor available in the outpatient unit.
  • Patients with referral letter will be registered with the doctor whom they have been referred to.
  • Patient desiring to be registered to any other doctor will have to give a written request.
  • The Receptionist will hand over the “Outpatient Registration Information Form to the patient/attender to fill in the patient’s demographic details which is digitalized. The system automatically generates a unique identification number and a bar-coded Outpatient Card (i.e. smartcard) is issued which is used throughout his/her association with G. Kuppuswamy Naidu Memorial Hospital. It is also used to retrieve the records in the future. The patient is advised to bring this card during review visits.
  • In the absence of the patient, if the attender wants to meet the consultant to get an opinion, he/she needs to follow the same registration process.
  • The Nursing Assistant of the respective department will verify the patient’s documents and will call the patients based on the appointments.

Review registrations

  • Patients can fix their appointments with department concerned at the reception desk
  • On review visit patients should bring their outpatient card & all medical reports for review

Investigations

  • Prior appointment can be availed for radiological investigations
